Certificate of Immunization
- 美国S Department of State requires all F-1 students to complete and submit 3 immunization forms: Certificate of Immunizations, Medical History, and Tuberculosis Screening Questionnaire.

- Once you have registered for classes you will have access to your myEGSC account and will be able to upload the Certificate of Immunizations in the Student Forms section.
健康 Insurance
美国.S. Department of State requires all F-1 students to maintain continuous health insurance coverage. 美国niversity System of Georgia has extended this requirement to F-1 students and offers a Student 健康 Insurance Plan (SHIP) through United 健康care. You will be automatically billed after you register for classes every semester. 的 health insurance fee, paid in August, covers August through December. 的 fee paid in January covers January through July. During orientation, we will discuss how the health care system works in the United States and how to use this health insurance plan in the event that you require medical attention. Please bring any questions you may have to orientation!
All students are required to purchase the University System of Georgia Student 健康 Insurance Program (USG SHIP) unless they have insurance which meets the university system requirements.
